Four dental implants for a whole new quality of life

In the past, the path to fixed dentures was often not an easy one. Depending on the individual dental situation, several treatment steps were necessary to achieve a satisfactory and permanent solution. Each operation was a burden in itself, not to mention the long time it took to get the final prosthesis in place, which could take up to a year.

In the case of an edentulous jaw or if the remaining teeth had to be removed, the treatment proceeded as follows:

  • In a first operation, the remaining teeth were extracted if necessary.
  • If the jawbone had already receded, bone augmentation had to be carried out, e.g. with the so-called sinus lift.
  • After six months, the implants were inserted, but they had to remain unloaded at first.
  • Only after a healing period of several months could the fixed dentures be inserted and adjusted.

During the long treatment period, the patient had to cope with a temporary denture. This was usually a removable full denture whose fit was optimal in the rarest of cases and often severely restricted the quality of life. Last but not least, the provision of a fixed denture was also a considerable cost factor, because the bone augmentation plus 6 - 8 implants added up to a price that was unaffordable for many people.

With the All-on-4 method, we can now offer you an innovative surgical procedure that saves time and money. With this form of treatment, we provide the patient with four implants in one session - significantly fewer than was previously common - and insert a resilient denture on the same day.

The All-on-4 concept: fixed dentures with only four implants


The All-on-4 method allows us to make an immediate restoration with only four implants per jaw. For comparison: in the past, at least six implants were needed in the lower jaw and eight in the upper jaw.

Also, the All-on-4 technique does not require any bone augmentation. Only a few hours after implantation, a temporary denture can be firmly screwed onto the implants.

Our patients therefore usually only have to undergo one operation, which can also be carried out under general anaesthetic or in twilight sleep if desired. The remaining teeth can be removed in the same operation.

A special implantation technique makes it possible to restore teeth in just one day. The rear implants are placed at an angle of 30 - 45 degrees, which allows us to avoid bone augmentation even if the bone mass is low. In addition, the All-on-4 technique gives us the opportunity to use longer and particularly stable implants that allow immediate loading with fixed dentures.

In advance, a temporary bridge is made individually for the patient in our dental laboratory. Only a few hours after the implantation, it is firmly screwed onto the implants. You can leave our practice with a complete set of teeth and strong, resilient teeth. After about three months, the implants are fully healed and the final bridge can be fitted.

The All-on-4 concept explained simply

Bone loss makes it difficult to fit implants


If teeth have been missing for a long time or inflammatory processes have attacked the jawbone, bone mass may be lost. This makes implant treatment more difficult because the dental implants no longer have a secure hold in the receded bone.

There are also structures that must not be damaged during implantation, for example, the nerves running through the bone. Especially in the posterior region of the upper and lower jaw, it is therefore often not possible to carry out a classical implantation without prior bone augmentation.

All-on-4 implantation at the right angle

With the All-on-4 method, we make optimal use of the remaining bone mass by inserting the posterior implants into the bone at an oblique angle. In this way, we avoid anatomical obstacles and make use of the often better preserved bone mass in the front of the jaw. Contact with nerves and the maxillary sinus is avoided.

The oblique implantation direction also allows the use of significantly longer and more stable implants than before. These can be loaded on the same day. Scientific studies have shown that the implants placed at an angle can also keep up with straight implants in terms of their functionality in the long term.

Immediate loading instead of long waiting times

Conventional titanium implants require a healing phase of about 2 - 3 months, after bone augmentation even up to 6 months. During this time a firm connection is formed between the bone and the implant, the so-called osseointegration.

We speak of immediate loading when this healing period is not waited for and the implants are immediately fitted with a fixed denture. This procedure offers some obvious advantages: you get a fixed, functional and aesthetically pleasing denture straight away, without having to spend many months with an ill-fitting temporary.

The All-on-4 method is particularly suitable for immediate restoration. Four stable implants distributed over the dental arch are sufficient to securely fix the denture. In the meantime, there are many scientific studies that prove a success rate comparable to that of classical implantation. The prerequisite is careful planning and precise execution by an experienced surgeon, which is always the case in our dental practice in Zurich.
